Below is a quick guide to becoming a Medical Cannabis Patient in the State of Colorado.

1Can I apply for a medical marijuana card?

  • Must be a Colorado resident with a valid Social Security number
  • Have a qualifying medical condition or symptom
  • Examined by a doctor with whom you have a bona fide physician-patient relationship

2Does my condition or symptom qualify?

Approved Conditions & Symptoms

Per the Colorado constitution, medical marijuana may be recommended for the following:

  • Cancer
  • Glaucoma
  • HIV or AIDS Positive

OR, the patient has a chronic or debilitating disease or medical condition that produces one or more of the following:

  • Cachexia
  • Persistent Muscle Spasms
  • Seizures
  • Severe Nausea
  • Severe Pain

3Meet with your physician

The previous list of state approved conditions and symptoms may qualify a patient for the use of medical marijuana as a treatment option. This recommendation must be certified by your doctor. The "Physician Certification Form" must be filled out and mailed in by your doctor.

*For parents acting as a "Caregiver" for your children, please note that 2 physician recommendations are required.

Application Checklist
Fill out the entire application form. Caregiver designation items are optional. If you get stuck on anything, don't leave it blank. Contact us and we will be happy to help you.
Completed by your doctor. Your physician certification must be completed by a qualified MD or DO
You will need to attach a copy of your Colorado ID. If you don't have a Colorado ID, submit a Proof of Identity and Residency Waiver and the required supporting documents.
*Proof of Identity and Residency Waivers are valid only for one year. Upon renewing your medical marijuana card, you must provide a Colorado driver’s license or photo ID. 
If you wish to designate a caregiver, you must submit a Caregiver Acknowledgement and a copy of your caregiver’s ID. 
A $15 check or money order (non-refundable application processing fee) made out to CDPHE (or a Fee Waiver/Tax Exempt Status form)
Mail: Application Processing, CDPHE HSV-8608, 4300 Cherry Creek Dr S, Denver, CO 80246-1530.
Deliver to drop-box: 710 S Ash St, southeast entrance, Monday-Friday, 7:00 a.m. to 6:00 p.m. The drop box is on the wall inside the first set of glass doors. Your paperwork must be in a sealed envelope. You will not receive a receipt. If you wish to have a receipt, please mail in your paperwork by certified mail
