Below is a quick guide to becoming a Medical Cannabis Patient in the State of Colorado.
1Can I apply for a medical marijuana card?
- Must be a Colorado resident with a valid Social Security number
- Have a qualifying medical condition or symptom
- Examined by a doctor with whom you have a bona fide physician-patient relationship
2Does my condition or symptom qualify?
Approved Conditions & Symptoms
Per the Colorado constitution, medical marijuana may be recommended for the following:
- Cancer
- Glaucoma
- HIV or AIDS Positive
OR, the patient has a chronic or debilitating disease or medical condition that produces one or more of the following:
- Cachexia
- Persistent Muscle Spasms
- Seizures
- Severe Nausea
- Severe Pain

3Meet with your physician
The previous list of state approved conditions and symptoms may qualify a patient for the use of medical marijuana as a treatment option. This recommendation must be certified by your doctor. The "Physician Certification Form" must be filled out and mailed in by your doctor.
*For parents acting as a "Caregiver" for your children, please note that 2 physician recommendations are required.

*Proof of Identity and Residency Waivers are valid only for one year. Upon renewing your medical marijuana card, you must provide a Colorado driver’s license or photo ID.
Deliver to drop-box: 710 S Ash St, southeast entrance, Monday-Friday, 7:00 a.m. to 6:00 p.m. The drop box is on the wall inside the first set of glass doors. Your paperwork must be in a sealed envelope. You will not receive a receipt. If you wish to have a receipt, please mail in your paperwork by certified mail
