1 | Visit your provider

During your appointment, ask your doctor to verify that all information on your physician certification is correct. Physicians can make updates to information on physician certifications at any time.

Your provider will submit the certification to the Registry electronically and will give you a copy for your records. You must finish the rest of the application process within 6 months of the date your provider certification was issued. You must submit a new provider certification each time you apply.

Physician qualifications

Your physician must meet the following requirements to recommend medical marijuana:
  • Physician must be in good standing and licensed to practice in Colorado. Contact Colorado Department of Regulatory Agencies for information about physician licenses
  • Physician must be an MD or a DO
  • Physician is registered to submit certifications online
Minor patients
  • All minor patients must see two providers and submit two physician certifications.
  • If the minor patient has a qualifying disabling medical condition, which includes Post Traumatic Stress Disorder and Autism Spectrum Disorders, and any condition for which a physician could prescribe an opioid, and the recommending physician is not the patient’s primary care physician, the physician should review the records of a diagnosing physician or a licensed mental health provider acting within his or her scope of practice. Qualifying medical conditions
Debilitating medical conditions
Cancer.
Glaucoma.
HIV or AIDS.
Cachexia.
Persistent muscle spasms.
Seizures.
Severe nausea.
Severe pain.

Disabling medical conditions
Post Traumatic Stress Disorder (PTSD).
An Autism Spectrum Disorder.
Any condition for which a physician could prescribe an opioid.
Residency requirements
Adults
To be eligible for a medical marijuana card you must:
Be a Colorado resident
Be 18 or older
Have a qualifying medical condition


Minors
Minors are eligible for a medical marijuana card if:
Minor and the primary parent are Colorado residents
Minor has a qualifying medical condition​

I am adult applying for myself
  • A valid Colorado ID or driver's license. If you are applying online, make sure to have a JPEG or PDF copy of your valid Colorado driver’s license or ID. You will upload this to your registration.
  • Your social security number.
  • Caregiver’s registration ID if you are applying with a caregiver.
  • Credit card or bank account and routing number to pay the $25 application processing fee.
I am a parent or legal representative applying on behalf of a minor
  • Primary parent’s Colorado driver’s license or ID.
  • Minor's social security number.
  • Primary parent's social security number.
  • Certified copy of the minor’s state-issued birth certificate.
  • Caregiver’s registration ID if parent is not the caregiver.
  • Credit card or bank account and routing number to pay the $25 application processing fee.
  • Documentation to prove secondary parent status:
    • Secondary parent’s out of state ID if parent lives out of state.
    • Copy of secondary parent’s certified death certificate if parent is deceased.
    • Copy of a certified court-issued sole custody order if the secondary parent does not have any custody.
  • Minor patient consent form
  • If you are applying online, make sure to have JPEG or PDF copies of all required documents.You will upload the documents to your registration.
I am a legal representative applying on behalf of an adult
  • Patient’s Colorado driver’s license or ID.
  • Legal representative’s Colorado driver’s license or ID.
  • Patient's social security number.
  • Legal representative's social security number.
  • Copies of certified legal representative documentation, medical power of attorney documents or certified court orders.
  • Caregiver’s registration ID if the legal representative is not the caregiver.
  • Credit card or bank account and routing number to pay the $25 application processing fee.
  • If you are applying online, make sure to have JPEG or PDF copies of all required documents.You will upload the documents to your registration.

3 | Complete your registration

No internet access? Apply by mail
Enter Patient Information
If you do not see your name, click “new” to enter your patient information. Your application will be processed as a renewal if you have applied by mail in the past.

If you see your name, click on it to start the renewal process.
Add provider certification
1. Click the “Provider certification” tab
2. Click “new”
3. Choose your provider certification from the drop-down menu
4. Click “save”
Enter Cultivation information
1. Click the “Cultivation information” tab
2. Click “new”
3. Select your cultivation option and complete required fields
4. Click “save”

You will be able to change your cultivation option once your card is active.
Pay
Use a credit card, debit card or electronic check to pay the $25 application processing fee. This fee is non-refundable and is required each time you submit an application.

4 | Check your email

You will receive a message telling you to log into your account.

5 | Log into your account

Click the “Notifications” tab to read messages.

6 | Print or display your card

Click the “print card” button to print your card or display it from a mobile device after you have been approved. If you applied online you will not receive a card in the mail.
